Form
If + past perfect, would have + III form/-ed (past participle)
In the would-clause you may use: would/could/might. 

Use
We use the 3rd conditional when we imagine a different situation in the past and its potential results (in the past). An imaginary situation in the past leads to potential imaginary results.

Examples
If the candidate hadn't arrived late, he would have been offered the job.
If you had checked the source of information, you wouldn't have been misinformed.
If I hadn't read the book, I wouldn't have known what the film was about.
We would have finished painting the kitchen if we hadn't run out of paint.
